Hey! I think I found the problem.
It seems as though once, long ago, I raised the file descriptor (FD) limits of Gir (or Zim) from the default (1024) to something much larger (4096 I think). But at some point that change got lost. This limited the web server's FDs and ultimately required that all new requests get queued up. So for a long time we've been running with one queuing thread, 200 KA threads, etc and so forth. What it boils down to is that what should have been asynchronous request processing became synchronous.
I've fixed the limited FD problem (we're back up to 4096 now), and I haven't been able to reproduce the problem. If other people still see the problem (it'll exhibit itself as either images/icons not being displayed, or the whole of the forum suddenly looking very 1995 in its layout), please let me know.
